Kohl version 3.02.05.000

When new catalogue entries are input, the date is recorded correctly but when reviewing the audit history. The dates appear as 2014 and shows that more than one librarian edited the entry although only one person has done any editing.

Are you sure, you are looking at the logs of the correct record? Sometimes itemnumbers and biblionumbers can get mixed up.
The version you reported this against is no longer maintained - please reopen if this still appears in a current version of Koha.

When you update, the database is updated as well. You can, for example, set up a new server with a current version, import your database, run the web installer that is triggered automatically and the database will be updated. It's recommended to do this on a test system/after a backup.
