To recreate:
Have EnableItemGroups and EnableItemGroupHolds enabled, with a record that has multiple groups on it and a corresponding circulation rule enabling unlimited holds per record.
From OPAC, place hold on one item group.
Go back to record to place hold on second item group. At the top of opac-reserve.pl is a message reading "You already have at least one record level hold on this title. All further holds must be record level."
Note that the hold you placed was not a record-level hold, and you can still go ahead and place an additional group-level hold. Hold type is inaccurate, and it doesn't actually block placing the hold.
