When making a reserve on an item, the system always claims that the item
already has one reserve.

Problem appears on this page in the OPAC: /cgi-bin/koha/opac-reserve.pl after
clicking a 'request' link.

Sorry for not being more specific. The problem happens when a patron uses the
online catalog to search for a title and then tries to reserve that title using
the "reserve this title" button. After the patron logs into Koha, the
page "reserve page for Item:xxx" claims that there is already one reserve on
