Can not choose the permissions

Dear Friends,
I'm begginer to Koha.
I just tried to install Koha 3.18.04.000 to Linux koha 3.2.0-4-amd64 #1 SMP Debian 3.2.65-1+deb7u2 x86_64 GNU/Linux (mysql Ver 14.14 Distrib 5.5.41, for debian-linux-gnu (x86_64) using readline 6.2).

The installation was error-free.

I just tried to alter patron permissions: click the 'More' button on the top right of the patron profile and choose 'Set Permissions', but I can not choose the permissions. (look at attachment)
I have bug in log (/var/log/koha/koha:
"Use of uninitialized value $roadtype in string at /usr/share/koha/intranet/cgi-bin/members/member-flags.pl line 176"

Hi annvpro, looking at your screenshot I think the issue is that some of the mandatory data got 'unchecked' while running the web installer or that you installed with a non-english installer and out of date files. Please bring this up on the mailing list if it's still an issue.
