User cannot add subscription to a bib if using Internet Explorer 7 as the user agent:

"I could never get a record added in IE7 because the numbering box for volume, number and rollover wouldn’t display. I didn’t test the radio buttons to see if I could select either one but can if you’d like. "

Screenshot attached to this bug highlights the problem, as well as two display issues:

[1] captions of some radio buttons and form inputs are compressed horizontally and stretched out vertically.

[2] Excessive vertical spacing between form inputs.

Volume info displays when pattern is selected.

Sending a patch for the functionality bug, including documentation for failed attempt at fixing label display.  

Class "widelabel" added to relevant labels.  Both (very high priority) CSS and jquery setting of width do not "take" in IE7. The background-color, however, can be affected.  Go figure.

After applying the patch, the "create an item record when receiving this serial," "do not create an item recodr when receiving this serial", "Staff and Public Display", "Number of Issues to display to Staff," and "Number of issues to display to the public" labels are all still too narrow. I did ctrl-f5 to force a complete refresh.

The problem where the numbering patterns were not appearing is fixed by the patch.

Pushed patch as it resolves the functionality problem.  Leaving bug open for the label display problem.
